Diabetes Prevention Programme
The Diabetes Prevention Programme is to support individuals who have been referred to Southmead Development Trust due to their blood sugar levels being in the pre-diabetic range, which their GP normally finds out when they go in for a routine blood test.
They receive a 1to1 assessment with us and then join a peer group to discuss issues around diabetes, diabetes prevention and how to make behaviour and lifestyle changes to reduce their risk of developing diabetes. The peer group meets weekly for 8 sessions and then 4 more times after that over a year to chart progress and to support change. It’s been really successful so far and we’re really excited to see more people make lasting changes to improve their health and wellbeing.
We partner with Westbank Friends (Living Well Taking Control) to run this.
